I would like the church to treat these donations as GIFT AID. I currently pay UK Income Tax at the Standard Rate or above. I will notify the charity in writing if this situation changes.

To qualify for GIFT AID you must pay at least the equivalent amount of tax in the same tax year. If you do not pay UK Income Tax or do not pay sufficient UK Income Tax to qualify, then tick the NO box.
If the account details given are for a joint account with only one party paying income tax, Gift Aid can only be reclaimed if the donation is from the tax payer.
